The 3 Steps

There are 3 steps that you need to complete before your appointment with us.

Step 1: Take Pictures of your eyes, tongue and face (instructions below)

Step 2: Create a Client Portal (instructions below)

Step 3: Upload Your Photos to the Client Portal (instructions below)


Step 1: Take Your Pictures:  Eyes, Tongue and Face

Below is the step by step video and written instructions on how to take your photos.


Eye Photos

Eye Photo Instructions:

  1. You can use the camera on your phone for this, a web cam or any other type of digital camera that you have.
  2. For all eye photos please bring your eye as close to the camera lens as possible without it being blury.
  3. Make sure you are in a well light area.
  4. You will be taking a total of 4 pictures for each eye.  Eight pictures all together.
  5. We are going to start with your right eye.
    1. Pull your right bottom eyelid down toward the bottom of your face.
    2. Look up.
    3. Make sure the camera is very close to your eye.
    4. Take the picture.
    5. Pull your right top eyelid up toward your hair line.
    6. Look down.
    7. Make sure the camera is very close to your eye.
    8. Take the picture.
    9. Using your thumb and index finger spread apart your top and bottom eyelid.  Opening up the top eyelid towards your hair line and the bottom eyelid is moved down towards the bottom of your face.
    10. Look to your left.
    11. Take the picture.
    12. Now look immediately to your right.
    13. Take the picture.
  6. The exact same process is performed for your left eye.

Tongue and Face Photos


Tongue Photo Instructions

  1. Get in a well lit area or use the flash and your camera.
  2. You may use a cell phone, web camera or any other digital camera that you’d like.
  3. Do not brush you tongue with a toothbrush.
  4. Do not rise your mouth out with water or mouthwash.
  5. Stick out your tongue as far as you can and take your picture

Face Photo Instructions

  1. Get in a well lit area or use the flash and your camera.
  2. You may use a cell phone, web camera or any other digital camera that you’d like.
  3. Take these pictures WITHOUT ANY MAKE UP ON.
  4. Center your face in the screen.  Don’t try and get a great selfie angle so you look good.
  5. DON’T SMILE although you may smize (smile with your eyes…LOL).
  6. Take the picture

Step 2:  Create a Client Portal

Below is the step by step video and written instructions on how to create your client portal and upload your photos

  1. Access the email we sent you.  It should be an email titled “Welcome to Your Patient Portal”
  2. Click the Optimantra hyperlink, which will contain the web address to set up your portal.
  3. Once directed to the login screen click New User and enter your personal information.
  4. IMPORTANT:  Make certain that the email address, your birthdate and the spelling of the first and last name that you gave to the AlternaCare employee that scheudled you  matches what is being entered into the profile or it will not work.  If it doesn’t work then you will cry and we will cry and all will be lost…ok, maybe a little over dramatic there but you get the point…LOL.
  5. Once this step is complete you are now able to access your portal.

Step 3:  Upload Your Pictures to the Client Portal

Above is the step by step video on how to upload your photos to your client portal and below are the and written instructions on how to upload your photos to your client portal.

  1. Once you have logged in your patient portal you will click on “Documents and Handouts” on left side of the screen.
  2. Photos must be labeled with your name and body part PRIOR to upload. Example:  Lisa Smith right eye looking up.  Lisa Smith left looking down.  Lisa Smith… I think you get it 🙂
  3. On the”Upload” tab click on “Choose File” and then select the photo you would like to upload.
  4. Once the file has been selected click on “Record Type” and select “Medical Record.”
  5. Click “Upload”
  6. Repeat steps 1-5 to upload multiple photos
